help me to make filler pdf

Dear sagar,

I have made a questionnaire (see attachment), wish to add filler in the multiple choice options (ka, kha, ga, gha ko aagadi). I tried using acrobat reader and internet as well. In addition, name/address (third page) also should be writable. I spent many hours but still couldn't make.

Please help me to convert it writable so that one can just click on best choices and write name and address and save it easily. 

Go to the attachment and suggest me.
